I just split the two after and reinstall the adapter then a filled filter.

Suggestion
I would make sure your torque wrench is "reasonably" accurate or try a different one known be accurate and "check torque" on the adapter.

So the last two times I have changed my oil after installing the LN oil filter adapter, as well as the magnetic drain plug, the adapter comes off with the filter when removing.

So the last two times I have changed my oil after installing the LN oil filter adapter, as well as the magnetic drain plug, the adapter comes off with the filter when removing. I have it installed with the correct torque of 18 ft/lbs and the filter installed hand tight + 3/4 turn. I have oiled the o-rings as well prior to installation. Is anyone else having issues similar? I just split the two after and reinstall the adapter then a filled filter. Thoughts?

Call LN Engineering customer service. They are usually very helpful.

It's happened to me twice in 8 years. Make sure to lubricate the filter gasket with fresh oil.
